Yizkor Book Form and Donations - 2017

Each year B’nai Havurah remembers our departed loved ones in our Yizkor Book. Yizkor means "may (G-d) remember".

For our members:
As a courtesy to all of our B’nai Havurah members, we include the names of their departed loved ones in the Yizkor Book. All members should have received a mailing from us that includes a reprint of your family's listing in last year’s Yizkor Book. If this is the first time your family will be remembering a loved one in our Yizkor Book, or if last year’s listing is incorrect or incomplete, please complete the form you received in the mail or fill out the electronic form below. If we do not receive a new/corrected form from you, we will assume that you wish your listing to remain the same. however, we ask that all members review the information mailed about their yahrzeits, log into their secured online account, or call the office to confirm that any additional yahrzeits are added. Donations are appreciated. 

For our guests:
If you are not a member of B’nai Havurah, we are more than happy to include the name(s) of your departed loved one(s) in our Yizkor Book. Please add the names of your loved one(s) on the yahrzeit form, below no later than September 12, 9:00 am.  To include your loved one(s) in our Yizkor Book, we request a donation of $36 or more per name. Your Yizkor contribution not only helps to defray the expenses we incur in printing the Yizkor Book; it also enables the success of religious, educational, cultural and social programs at B’nai Havurah.
